Medicare Claims Under The Enhanced Primary Care Scheme

Your Health Domain Physiotherapists are recognized to treat clients on the Enhanced Primary Care (EPC) Scheme. This scheme has been established for the treatment of chronic medical conditions and patients who require complex care.

A chronic medical condition is one that has been (or is likely to be) present for six months or longer. It includes, but is not limited to, conditions such as asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke.

Patients have complex care needs if they require ongoing care from a multidisciplinary team consisting of their GP and at least two other health or care providers.

To find out if you are eligible for the EPC Scheme, speak to your General Practitioner. If your condition is deemed appropriate for the EPC scheme your GP will complete a GP management plan and a referral for physiotherapy. This may make you eligible to receive up to 5 physiotherapy treatment sessions, which will entitle you to receive a partial refund from Medicare.

You need a GP referral to attend physiotherapy under the EPC scheme. You cannot self refer for treatment under a Medicare claim.
